Our client based in Derby is seeking an experienced Social Worker to join their Learning Disabilities and Transitions Service. This is an excellent opportunity for a dedicated practitioner to support young people transitioning into adulthood, alongside adults with learning disabilities and complex care and support needs.

The successful candidate will work closely with individuals, families, carers, education providers, health professionals, and partner agencies to deliver strengths-based and person-centred interventions that promote independence, wellbeing, and positive outcomes. You will play a key role in supporting individuals through key life transitions, ensuring they receive the right support at the right time whilst maximising choice, control, and community inclusion.

This role requires a confident Social Worker with experience of Adult Social Care, learning disabilities, transition planning, safeguarding, and multidisciplinary working.

What will your responsibilities be?

• Managing a caseload of young adults in transition and adults with learning disabilities and complex support needs.

• Undertaking Care Act 2014 assessments, transition assessments, reviews, and support planning to identify eligible needs and achieve positive outcomes.

• Supporting young people and their families through the transition from Children's Services into Adult Social Care.

• Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and community participation.

• Working collaboratively with education providers, health professionals, Children's Services, carers, families, and partner agencies to ensure coordinated support.

• Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.

• Leading and contributing to safeguarding enquiries, ensuring adults at risk are protected from abuse, neglect, exploitation, and self-neglect.

• Assessing and managing risk appropriately whilst promoting positive risk-taking and individual choice.

• Supporting individuals to access community resources, supported living opportunities, education, employment, and meaningful daytime activities.

• Coordinating support arrangements for individuals with complex needs and ensuring timely reviews of services and care packages.

• Participating in multidisciplinary meetings, safeguarding discussions, transition planning meetings, and case reviews.

• Preparing high-quality assessments, reports, support plans, and professional documentation.

• Maintaining accurate, timely, and legally compliant case records in accordance with organisational policies and statutory requirements.

Qualifications & Experience:

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.

• Registration: Must be registered with Social Work England.

• Experience: Significant post-qualified experience within Adult Social Care services.

• Learning Disabilities Experience: Experience supporting adults with learning disabilities and complex care and support needs.

• Transitions Experience: Experience working with young people transitioning from Children's Services into Adult Social Care is highly desirable.

• Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.

• Assessment Skills: Proven ability to undertake transition assessments, Care Act assessments, support planning, and risk management interventions.

• Safeguarding Experience: Experience managing safeguarding concerns and undertaking safeguarding enquiries.

• Multi-Agency Working: Strong experience working collaboratively with health services, education providers, Children's Services, carers, families, and partner agencies.

• Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.

• Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.

• Organisational Skills: Ability to manage competing priorities and work effectively within a busy multidisciplinary service.
